An Online Protest-Form based on Google Forms

I was working on another Google Forms project (for racing) and decided to see what an online version of a Protest Form would look like .. so here it is.  This is definitely a 'work-in-progress'. 

It is purposely unbranded, ad-free and free to use as-is or mod for your own purposes.  I created a gmail account called protestform@gmail.com to host the form and the email script, so the resulting emailed-form will be emailed-from this gmail account (but the sender's name will be the "Name of Representative" supplied in the form).

About the Form
The 1st Q is the email address in the SI's to submit the form (field is optional).  If that is left blank, it will only send a copy to the submitter.  This would be handy if they wanted to attach a drawing and then send it in.  If you want to test both the copies being sent, you can use a 2nd email of your own to test at first.  The main email goes to the filer and a "cc" is sent to the SI email address if provided.

The idea was to create an email that could both be printed and used like a paper form or just forwarded around via email.  For that purpose, at the top of the email I included "logging" info so that if it was printed, it could be filled out by hand.  Also at the  end of the form, I provide an opportunity to record a withdrawal of the protest.

The form looks/works equally well on a mobile browser or a computer.  The subject line of the email will tell you what type of filing it is and the event name.

The form is "live" now .. in that it will send out emails to the addresses provided.  Therefore, if you actually want to use it as a competitor or YC, please feel free. 

If you'd like to have/customize this for your YC and regattas and want to hard-code the form-submission email address defined in the SI's (thus removing the 1st question), that would be really easy to do.   Send me an email privately and I'll share a copy of the form that you can edit.  Otherwise, you can just use the link below and have users fill-in the SI-email-address manually.

PS .. I coded the script so that email sent has a "replyTo" field set to the filer's email address.  Therefore, if the YC receiving the email .. replies to the email .. the emailed-reply will automatically be addressed to the filer .. NOT to protestform@gmail.com.  Test that out for yourself by replying to the email you receive and you should see the "to" field populated with your email address. - Ang

Update: I added an email auto-responder that sends a link to the form.  If you send an email to protestform@gmail.com with word "form" in the subject (it could be "form", "protest form", etc) it sends you a reply with the link to the form. - Ang

I would love to see the following additions:
In the type of hearing, I'd love to see "Scoring inquiry/redress"
"When/Where did the incident occur?"

Thank John .. normally a scoring inquiry is separate and goes to the race committee instead of the protest committee.  I have written a separate scoring inquire Q&A  though that I could makr avail too.
